All participants in USA Yoga Events (Both Virtual and Live), including but not limited to the Galt House Hotel in Louisville, KY, and Indiana Bikram Yoga in New Albany, IN must read and agree to the following Release and Waiver of Liability and Video Release in order to register for any event:

RELEASE AND WAIVER OF LIABILITY: I am participating in a workshop/event (Activity) offered by USA Yoga. I recognize that I must be in adequate physical and mental health to participate; this activity may require intense physical exertion and I represent that I am physically fit enough to participate, and I have no medical condition which would prevent my full participation in this activity. I recognize this activity may cause or aggravate a physical injury or medical condition, I understand it is my responsibility to consult with a physician before my participation. I am aware that my participation could result in certain physical symptoms or injuries; I understand my physical limitations and I am sufficiently self-aware to stop or modify my participation before I become injured or aggravate a pre-existing injury. In consideration of being permitted to participate in this activity, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I may incur as a result of my participation. I expressly waive any claim I may have against USA Yoga, USA Yoga Board of Directors, USA Yoga Contractors, and participating instructors (Released Parties). I agree to indemnify and hold harmless each Released Party from any loss, cost or liability incurred. Claim includes but is not limited to any and all liabilities, claims, demands, expenses, fees, legal actions, right of actions for damages, personal injury, mental suffering and distress in connection with participation in this activity. I acknowledge that I have carefully read this agreement and fully understand its contents. I voluntarily and knowingly agree to the terms and conditions stated herein. I am aware that by signing this agreement, I am giving up substantial rights, including my right to sue and certain legal rights my heirs, next of kin, executors, administrators and assigns may have against any Released Party.

PARENT GUARDIAN WAIVER FOR MINORS UNDER 18 YEARS OLD: Yes, by checking this box I understand that I agree to all the terms and conditions stated in the above Agreement regarding "Waiver" and this constitutes an electronic signature that certifies that I have read, understood, and agreed to my rights and responsibilities, and I also understand this Agreement becomes a legally binding instrument when I check this box entered into by me and USA YOGA..The undersigned parent and natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.

VIDEO RELEASE:  I hereby grant permission to USA Yoga the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or digital video. I understand that my image may be copied, exhibited, published or distributed and I waive the right to royalties or other compensation arising or related to the use of my image or recording. By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in a public setting. I further understand that in the event that I am participating in a non-competitive online workshop or class, I have the option to turn off my video camera if I do not wish for my likeness to be shared. There is no time limit on the validity of this release. This release applies to the video recordings collected as part of the session listed on this document only. By signing this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against USA Yoga or its representatives using this materials for legitimate purposes.
